- comments: observed no valid city of chicago sanitation certificate on site at time of inspection. Instructed person in charge valid city of chicago food sanitation manager certificate or proof of training is required on site at all times tcs foods are being prepared and served. Priority foundation 7-38-012. Citation issued.

- comments: observed build up on interior walls of ice machine. Instructed person in charge to clean and maintain.

- comments: observed fly paper and fly "zapper" in basement area. Instructed person in charge to remove and request service for lying insect abatement from licensed pest control service.

- comments: observed no test kit for low temperature dish washing machine and three compartment sink sanitizer on site. Instructed person in charge to provide and maintain test kit for three compartment sink (quat sanitizer), and low temperature dish washer(chlorine sanitizer) and maintain at all times. Priority foundation 7-38-005. Citation issued.

- comments: observed no self closing device on men's room door. Instructed person in charge to install self closing device and maintain.

- comments: observed mechanical ventilation in restrooms not operable at time of inspection. Instructed person in charge to repair and maintain.
