39. Contamination prevented during food preparation, storage & display
- comments: observed boxes of beverages stored on the floor in the dry storage/equipment storage room located across from the utility sink closet in east hall. Instructed manager to elevate all items at least 6 inches off floor.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed ice machine with upper compartment lid to be damaged with the lid hanging off and being held in place with duct tape. Instructed manager to service lid and maintain in good repair.

49. Non-food/food contact surfaces clean
- comments: observed shelves and inner bottom shelves in the cabinets in the bar area to be heavily soiled with plastic, trash, and loose pieces of paper and dust. Instructed manager to remove trash from all cabinet interior shelves.

51. Plumbing installed; proper backflow devices
- comments: observed leak at base of faucet of 3 compartment sink in main kitchen at time of inspection. Leak even when not in use. Instructed manager to service and maintain in good repair.

55. Physical facilities installed, maintained & clean
- comments: observed rear storage area behind curtains in "green room" and closet next to rear exit to the right of door to be crowded with tables, chairs, and boxes stored, not allowing access to clean floor. Instructed manager to organize and remove any items not necessary to function of facility.

- comments: observed mop sink to be heavily soiled with black build up on internal sides. Instructed to detail clean and maintain free of excessive build up.

60. Previous core violation corrected
- comments: previous core violations not corrected from 10/18/2019, report #2316078: #47) broken rubber gasket inside the two reach-in cooler doors was repaired with duct tapes. Instructed to replace said gasket. Also no duct tape allowed. Surface must be smooth,cleanable and non-absorbent material. #47) piece of wood under bar counter were gun pop holder is installed. Not smooth and cleanable. Repair cause surface must be smooth and cleanable. #51) no back flow prevention device provided at ice machine water line,must install so back flow prevention device may be located to be serviced and maintained. #53) no lids provided at receptacles inside the washroom at east hallway. Instructed to provide #55) vent cover above the ice machine with excess black dust build-up in prep area. Clean and maintain #55) debris on floor under pipes at bar area, clean and maintain. Priority foundation violation: 7-42-090, citation issued.
