- comments: observed no ambient air thermometer inside the prep cooler in the rear prep area. Instructed to provide and maintain.
38. Insects, rodents, & animals not present
- comments: observed approximately 15 mice droppings and 3 ants on site. Found mice droppings on the floor in the mop sink closet, under the shelves in the rear storage area, on the lower shelves under the cooking equipment, under the coffee equipment, and on the tabletop next to the microwave in the food prep area. Ants observed on the shelves where dry goods are stored and on the floor in the seating/ dining area. Instructed to remove mice droppings, eliminate live ants, clean and sanitize all affected surfaces. Priority foundation violation 7-38-020(a), citation issued

- comments: observed employee in rear prep area without hair restraint. Instructed to provide and maintain.

- comments: observed grocery bags being used as food storage bags for tcs foods observed in the freezer chest and also found rice stored inside a grocery bag stored inside the microwave. Instructed to use adequate food storage bags/containers.

- comments: observed the interior of the microwave with slight debris. Instructed to clean and maintain.
