- comments: observed no hand washing signage in the front toilet room. Instructed to provide.

- comments: observed evidence of pest activity in the following areas: rear garage/storage area approx 15-20 mice droppings along the walls and in corners. . . The rear reach in cooling unit area behind the equipment and corners by the rear exit door approx 20 droppings. . . The rear storage area approx 10-15 mice droppings in the corners near the door and approx 5 behind the deep freezer. . . And approx 10-15 behind the cooling unit in the front area across from the cash register. Instructed to clean and maintain all areas, contact pest control and have a service prior to return inspection date of 10/08/2025. Priority foundation violation 7-38-020(b) citation issued.

- comments: observed a heavy build up of grease and food debris on the sides of the cooking equipment in the food service area. Instructed to clean and maintain all as a means of preventing pest and insect activity.

- comments: observed the staff toilet room in the rear food prep area without a self closing device. Instructed to provide as door should close on its on helping staff prevent possible contamination from touch the door handle after washing hands.

- comments: observed floors and walls under and around all cooking and non cooking equipment with a build up of dirt, dust and wall splatter. Instructed to clean and maintain areas.
