- comments: 4-601. 11(c) observed the inside of the deep fryer (rear) oil filtering unit with a build-up of grease and food debris. Noted excess food debris on all surfaces of heavy equipment throughout front and rear prep/dishwashing area. Instructed to clean to prevent pest harborage.

- comments: 5-205. 15 observed hand sink piping leaking and pooling on the floor in the toilet room on right-hand side
55. Physical facilities installed, maintained & clean
- comments: 6-201. 11 observed excessive amounts of duct tape on some ceiling tiles as well as on walls throughout the prep area and window in the front prep area. Noted water-stained ceiling tiles in halway outside toilet rooms, front and rear prep/dishwashing, dining area and toilet rooms. Instructed to remove said tape, repair or repair said tiles. Must provide smooth and easily cleanable surfaces as required.

Violation states '5-501. 15 observed outside receptacle with all lids missing during time of inspection. Instructed person in charge to provide receptacle with tight fitted lids in order to prevent pest harborage. Priority foundation violation. #7-38-020 (b). Citation issued. Instructed to correct. Priority violation. #7-42-090. Citation issued.
