- comments: observed no covered receptacles in girls and unisex washrooms. Instructed manager to provide covered receptacles in all girls and unisex washrooms.
55. Physical facilities installed, maintained & clean
- comments: observed openings around pipe fittings along lower walls in kitchen near hand washing sink, damaged and missing wall tiles and baseboards along walls in kitchen, peeling paint along walls in rear storage area near walk-in freezer and dining area. Instructed manager to repair/replace and maintain.

- comments: previous priority foundation violation not corrected from report #2627170 on 11-17-25:60- previous core violation not corrected from report #2603769 on 10-1-24:38-observed the rear exit door in the room with the walk-in freezer area not rodent proofed at the bottom. Instructed to rodent proof said door and maintain. Priority 7-42-090 citation issued
