- comments: no signed employee health policies on premises as requried; must provide and maintain at all times. Priority foundation violation 7-38-010 citation issued.

- comments: found no pest control log book on premises as required for review with necessary documents and reports inside; instructed to provide and maintain. Priority foundation 7--38-020(c) citation issued.
49. Non-food/food contact surfaces clean
- comments: instructed to clean the refrigeration unit in the food prep area, clean the surface areas of the counter top in food prep area, clean blade of can opener at the prep table in the prep area, clean fan covers inside of walk-in cooler and shelves inside of walk in cooler, clean shelving on selling floor with prepackaged food items. Clean the exterior surface of the grease receptor and maintain at the three comp sink.

- comments: observed standing waste water in the mop bucket in the rear dishwashing area; instructed to dispose of water after using.

- comments: instructed to clean the floors and walls around under and behind equipment on the selling floor, prep area, storage area and walk-in cooler and maintain. The walls, floor and ceiling vents in the washroom need cleaning.
