- comments: observed several handwashing sinks throughout the facility missing signage notifying food employees to wash hands. Instructed to provide adequate signage to all handwashing sinks and maintain at all times.

- comments: observed insect control device installed directly above server food prep station in the rear prep area. Instructed to relocate insect control device such that it does not pose risk of contamination of open food, equipment, or utensils and maintain.

- comments: observed pest control log book on-site incomplete, missing numerous pest control service receipts from 2025 (excluding september and january 2025). Instructed to provide pest control log book with all items required by the law including all pest control service receipts and maintain available on-site for inspector review at all times. Person-in-charge printed all missing pest control service receipts and added to pest control log book at time of inspection. Priority foundation violation. 7-38-020(c) citation issued.

- comments: observed exterior nonfood-contact surfaces of ice machine to be soiled with build up of pink, slime-like substance and other soils. Instructed to clean nonfood-contact surfaces of equipment at adequate frequency to prevent build up of substances and other soils and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed plastic and aluminum foil used to line output of dish machine drain plumbing to act as splash guard, plastic and foil visibly soiled. Instructed to remove plastic and aluminum foil and replace with a splash guard made of a material that is smooth and easily-cleanable and maintain.

- comments: observed faucet at the rear kitchen area 1-compartment food preparation sink to be in poor repair, leaking when water is turned on. Instructed to repair or replace 1-compartment sink faucet to eliminate leak and maintain all plumbing fixtures in good repair.
