- comments: observed personal food item stored inside walk-in cooler, prep area cooler and rear storage area. Instructed to remove and provide a designated area where employees can store personal food items.

- comments: observed no ambient air thermometers inside the reach in coolers and display coolers. Instructed to provide to properly monitor the ambient air temperature of equipment.

- comments: observed food storage containers without labels containing the common name of foods. Instructed to provide.

- comments: observed no splashguard at the handwashing sinks in the kitchen/prep area near the mop sink and near the 1-compartment prep area sink. Instructed to install a washable splashguard.

- comments: observed plates not inverted. Instructed to invert plates after sanitizing and air drying.

- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish/replace.

- comments: observed wall with huge opening where drywall was removed above the food prep/dishwashing area. Observed plaster/drywall and wall tiles broken/cracked at/near the frontline handwashing sink and frontline prep area table. Instructed to replace drywall and broken/cracked wall tile.

- comments: observed city of chicago food service sanitation manager on duty without food allergen training. Instructed to provide certificate.

- comments: observed continued non-compliance from previous report #2620628 conducted on 7/15/25. Previous core violation not corrected for item #47) 4-402. 11 clean utensil rack next to mop sink, instructed to install a barrier to prevent splashing from mop sink. Priority foundation violation. 7-42-090. Citation issued.
