- comments: no city of chicago food service manager on duty as required. Instructed that a city of chicago food service manager must be on duty at all times tcs foods are being prepared and served. Priority foundation violation 7-38-012 issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: unable to produce the employee health policies for employees as required; must provide. Priority foundation violation 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: unable to provide the vomit and diarrhea clean up procedure and kit as required; must provide. Priority foundation violation 7-38-005.

10. Adequate handwashing sinks properly supplied and accessible
- comments: no exposed handsink behind bar as required for handwashing. Must install a hand washing sink behind bar with hot and cold running water, soap and handtowels. Priority foundation violation 7-38-030(c) issued. Handsink was removed. Previously violated on july 21, 2020 to repair leak from the pipe underneath the sink.

- comments: instructed to clean the exterior of the fryer in kitchen. Clean bottoms of prep tables.

- comments: instructed to provide a cover for the subpump in basement on the floor. Instructed to clean the floors under around and behind kitchen equipment and in basement area. Instructed to clean walls around and behind cooking equipment in kitchen. Clean walls in walk-in cooler and fan covers to remove dust observed. Clean fan covers in walk-in cooler to remove dust observed.

- comments: observed the ceiling in the walk-in cooler with exposed foam; must provide proper covering (panel) on the ceiling to cover foam.

57. All food employees have food handler training
- comments: observed no food handler training certificate for food handler in kitchen as required; must provide.

58. Allergen training as required
- comments: instructed that the city of chicago food service manager must obtain the required food allergen training.

60. Previous core violation corrected
- comments: previous core violations not corrected from july 21, 2020 as required: #53 the trash receptacle in the womens toilet room are without lids; instructed to provide trash receptacles with lids. #55 basement area is cluttered; instructed to remove all unnecessary items and organize the area. Priority foundation violation 7-42-090.
