- comments: observed no handwashing signage at exposed handwashing sinks in both bars. Instructed to provide.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed a low-temperature dish machine located in the rear dish area, maintaining an improper chemical sanitizer concentration of 0 ppm of chlorine. The machine was tagged and held for inspection. Instructed to repair machine so that it is capable of delivering proper chemical sanitizer concentrations between 50-100ppm of chlorine. Priority violation 7-38-025. Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed heavy build up of black and pink slime in the interior of ice machine located in basement used to produce ice for human consumption. Instructed to remove ice, clean, sanitize, and maintain unit free of foreign substances. Priority foundation violation 7-38-005. Citation issued.

- comments: observed exposed handwashing sink in rear dish room and faucet of same sink are loose. Instructed to correct and maintain in good repair.

- comments: observed no backflow prevention device at the utility sink located in the basement. Instructed to install following all relevant laws, rules, and regulations.

- comments: observed no covered trash receptacles inside both unisex toilet rooms located on the west side of the building. Instructed to provide.

- comments: observed build up of grease on exterior of used cooking oil container located in alley. Instructed to clean and maintain in good condition.
55. Physical facilities installed, maintained & clean
- comments: observed build-up of grease and food debris in the interior of fryer cabinets located in the food prep area. Instructed to detail clean and maintain in good condition.
57. All food employees have food handler training
- comments: observed one food employee without food handler training. Instructed all food employees must complete food handler training and maintain records thereof on site at all times.
