- comments: observed approximately over 30 rat droppings in the storage closet located in the basement where the heat water tank is located. ---observed approximately over 20 small black flying insects in the bar area. The facility must ensure all rat droppings and small black flying insects are removed from the premises. All areas are cleaned and sanitized, and a licenced pest control company treats the facility for rodent activity. Priority foundation violation 7-38-020 (a) citation issued.

- comments: unnecessary items / nonfunctional equipment were found next to the kitchen grill (unused fryer), storage closets where the heat water tank is located, and basement office/storage area. The premises shall be free of items unnecessary to the operation or maintenance of the establishment. This includes equipment that is nonfunctional or no longer used.
