22. Proper cold holding temperatures
- comments: observed approximately 10 dozen eggs on counter next to stove at 77f. All eggs were discarded. Priority violation 7-38-005 citation issued

25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory disclosure/reminder indicated on menu of raw/under cooked foods, for customer view. Informed manager that the disclosure/reminder statement and indicator must be in plain view of the customer, indicated on menu of raw/under cooked foods. Instructed manager to provide consumer advisory signage, disclosure/reminder indicated. Maintain at all times. Priority foundation violation # 7-38-005 no citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 50 rodent droppings in sump pump room off or basement food prep area. Must remove all rodent droppings and clean and sanitize all affected areas. Priority foundation 7-38-020 (a)

39. Contamination prevented during food preparation, storage & display
- comments: observed dry storage and food related items stored under sewer lines in basement. Relocate all food and food related items from under sewer lines.

41. Wiping cloths: properly used & stored
- comments: observed dirty wiping cloths on counters in cooking area. Must store wiping cloths in sanitizer when not in use.

49. Non-food/food contact surfaces clean
- comments: observed inside of fryolator very unclean with excess grease accumulation. Clean and maintain inside of fryolator.

52. Sewage & waste water properly disposed
- comments: observed excess water on floor of womens washroom in basement. Must eliminate all water and keep floor clean and dry.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outdoor grease accumulation on top of grease container. Clean and maintain grease container.

55. Physical facilities installed, maintained & clean
- comments: observed raw wood wall in basement prep area needs to be painted or sealed so that it is non-absorbent so that it is smooth and easily cleanable. Observed holes in walls at base of sump pump room. Repair walls and maintain. Observed dry storage room next to bathrooms in basement with walls and ceiling in disrepair. Repair walls and ceiling and restore to a smooth easily cleanable condition and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed vent in washrooms and vent above mop sink need to be cleaned of heavy dust accumulation and maintained.

- comments: proof of food handler training for employees did not be provided. Must provide proof of food handler training food all employees.
