- comments: observed several dead cockroaches throughout establishment on floors and glueboards. Must remove.

- comments: observed baseboards and open cracks and crevices throughout facility not sealed located in 1st floor kitchen prep/dishwashing area, basement prep area, bar and rear hallway space leading to outdoor garbage area. Instructed to seal all baseboards, cracks and crevices.
59. Previous priority foundation violation corrected
- comments: previous priority foundation violation not corrected from 12/4/2025, report #2628065. #38--observed over 7 live cockroaches throughout establishment. Live cockroaches were found at the bar area, kitchen prep area 1st floor and in prep area in basement. Must eliminate all evidence of pest activity on premises. Must clean and sanitize all affected areas. Priority violation. 7-42-090. Citation issued.
