38. Insects, rodents, & animals not present
- comments: observed over 150 to 200 mouse droppings along the walls in the front (east) storage areas, on the shelves throughout the dry storage areas, in between boxes on the north and south storage shelves, in the storage closet on the northeast corner of facility, and in both employee bathrooms. Instructed manager to clean, sanitize, and maintain areas free of mouse and rodent droppings, and recommended pest control service facility prior to reinspection. Priority foundation violation, 7-38-020(a). Citation issued.

- comments: observed seal of garage door in rear exit/delivery area to have a gap larger that 1/4 inch along the bottom of the garage door seal. Instructed to pest proof the garage door and seal approproately.

39. Contamination prevented during food preparation, storage & display
- comments: observed boxes of food and cans stored on the floor next to shelves in the dry storage areas throughout facility. Instructed manager to elevate all items at least 6 inches off floor.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cardboard used to line shelves, carts, and pallets throughout facility storage shelves, in the kitchen prep area, and in the dish washing area. Instructed managers to remove and provide a smooth and easily cleanable surface.

51. Plumbing installed; proper backflow devices
- comments: observed two (2) of four (4) handles at the 3 compartment sink to be missing/not attached at time of inspection. Employees still using sink with hand turning the spindle, and hot and cold water is still available. Instructed to service 3 copmartment sink handles and maintain in good repair.

- comments: observed mops stored both in a bucket submerged in water and stacked and soaked on top of each other in mop sink. Instructed manager to hang and store mops to allow for air drying in between use.

- comments: observed the storage closet located near the entrance doors on the north east corner to be heavily cluttered with various equipment, boxes, paint cans, and unused equipment. Instructed to remove and maintain area free of unneccesary equipment to function of facility and reduce areas for pest harborage.

- comments: observed floor below and behind ovens and fryer equipment in southwest corner to be heavily soiled with grease and food debris. Instructed manager to clean and maintain areas free of excessive grease and food debris build up.

- comments: obsered three (3) of five (5) lightbulbs in ventilation hood above ovens and fryer area to not be functional at time of inspection. Instructed manager to repair or replace to ensure adequate lighting in area.

- comments: previous core violations not corrected from 08/23/2023, report #2580546: #53) 6-501. 19: - observed both toilet room doors not self-closing. Instructed the person in charge to provide a self-closing mechanism for both toilet room doors and maintain. #55) 6-501. 11 - observed hole in ceiling in the electrical room in the rear. Instructed the person in charge smoothly to seal the hole and maintain it in good repair. Priority foundation violation: 7-42-090, citation issued.
