- comments: observed no written procedure posted on the premise of neccesary items outlined in policy available during time of inspection for responding to vomit and diarrheal events that can occur on premise. Instructed to provide and maintain. Priority foundation violation. #7-38-005. Citation issued

- comments: observed evidence of pest activity on the premise during the time of inspection. Noted approximately 4 live cockroaches in prep area on floor near hot water tank. Recommended to contact pest control for service. Must eliminate pest activity. Priority foundation violation. #7-38-020 (a). Citation issued.

- comments: observed heavy equipment on the 1st floor serving area in need of cleaning. Instructed to remove food debris on steam/prep table shelving. Must be maintained while not in use.
