- comments: 2-101. 11 observed no city of chicago food service sanitation manager on site while tcs foods (deli meat) are being served to the public. Instructed a city of chicago food service sanitation manager must be on site at all times the facility is open, operating and selling open tcs foods to the public. Prioirity foundation. - comments: 2-102. 14(o) observed no employee health policy on site. Instructed to provide at all times. Priority foundation. Citation issued 7-38-010.
38. Insects, rodents, & animals not present
- comments: 6-202. 15 observed the front and rear doors with a 1/2 inch gap at the bottom of both doors. Instructed to make said doors tight fitting and rodent proofed.

- comments: 6-501. 114 observed unnecessary clutter in the rear storage area. Instructed to remove clutter, organize and elevate everything at least 6 inches from the floor.
57. All food employees have food handler training
- comments: 2-102. 13 observed no food handler certificates on site. Instructed to provide.
59. Previous priority foundation violation corrected
- comments: 8-404. 13(b:3) observed previous priority foundation violation not corrected from inspection report #2616854 dated 5/5/25; 10 - 5-204. 11 observed no hand washing sink in the food prep area. At the time of the inspection, there is a food prep area that the facility created despite being licensed as a risk 3 facility on 5/5/23. Instructed to install a hand washing sink in the food prep area. 36-4-302. 12 observed no metal stem thermometer on site to properly take temperatures of cold and hot foods. Instructed to provide at all times. 38 - 6-501. 111 observed evidence of pest activity on site. Observed about 10 rodent droppings in the chip boxs located on the lower shelf of the chip rack. Observed about another 10 rodent droppings located in the rear storage area under shelves. Instructed to eliminate rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. 48 - 4-301. 12(a) observed no 3 compartment sink in the facility. Instructed to install a 3 compartment sink. Instructed to correct all previous priority foundation violations. Priority violation. Citation issued 7-42-090.
