- comments: observed no city of chicago foodservice manager certificate posted on site at time of inpsection. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no verifiable employee health policy on site at time of inspection. Left template and instructed to maintain copies of employee health policy signed by all employees on site at all times. Priority foundation violation 7-38-010. Citation issued.

- comments: observed no soap inside employee toilet room. Instructed to provide soap at all handwashing sinks at all times. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed torn gaskets on interior of prep cooler doors and walk-in cooler door. Instructed to repair or replace and maintain in good condition.

- comments: observed no chlorine test strips on site at time of inpsection. Instructed to provide. Priority foundation violation 7-38-005. Citation issued.

- comments: observed slight leak coming from top pipe of urinal in first floor men's toilet room when urinal is flushed. Instructed to repair or replace and maintain in good condition.

- comments: observed no covered trash receptacles inside unisex employee toilet rooms and upstairs unisex employee toilet rooms. Instructed to provide.

- comments: observed outdoor dumpster in rear alley overflowing with garbage causing lid to be unable to be shut. Instructed to correct and maintian outdoor garbage dumpsters with tight fitting lids. Priority foundation violation 7-38-020(b). Citation issued.

- comments: observed missing ceiling panel above rear dish area. Instructed to correct and maintain in good condition.

- comments: observed cracked light shield in ceiling above rear dish area. Instructed to correct and maintain in good condition.

- comments: observed no food handler training certificates on site at time of inspection. Instructed all food employees must complete food handler training and maintain records thereof on site at all times.

- comments: observed the following repeat core violaiton not corrected from report #2373845 dated 6/17/2020: #51 'observed no 1 comp sink behind the service bar on the second floor; must install with hot and cold running water. ' instructed to correct and come into compliance. Priorty foundation violation 7-42-090. Citation issued.
