- comments: observed an infestation of appx. 200 or more mice droppings scattered throughout premises on shelving units and floor at food items, dog & cat food, baby items, service counter and registers, opening in rear storage wall frame, and a dead mouse on glue board in the same area. Also, evidence of a live small mouse in between bags of dog food on the lower shelf. Must remove droppings and dead mouse. Clean and sanitize areas. Must have a pest control operator to service premises on or after 6/5/25. (priority 7-38-020(a)(citation issued)

- comments: observed a gap along bottom and center of front double entry door must make tight fitting to prevent rodent entry.

Instructed to provide. : #55-6-501. 11 : observed floors and ceilings throughout the facility are cracked and broken. Instructed to repair or replace and maintain in good condition.
