- comments: observed no city of chicago certified food manager on site upon arrival. Instructed manager to ensure a city of chicago certified food manager is on site at all times. Priority foundation violation 7-38-012 citation issued.

- comments: observed hot tofu and chicken held at an inadequate temperature of 110 f. Manager stated time as a control was being used however there was no documentation recorded for the day. Instructed manager to record time as a control properly or keep hot food at 135 f and above. Food rapidly reheated to 165 f. Priority violation 7-38-005 citation issued.
38. Insects, rodents, & animals not present
- comments: observed more than 15 small flies in kitchen of facility. Instructed manager to service all areas affected by pests. Pest control service recommended. Priority foundation violation 7-38-020(a) citation issued.
