- comments: 6-301. 14--observed no handwashing signage at rear prep handsink adjacent to restroom and front prep area. Instructed to post handwashing signage at all handsinks throughout premises and maintain.

- comments: 3-302. 12--oberved bulk containers of flour, sugar, margarine without labels. Instructed to label all bulk containers with common name and maintain.

38. Insects, rodents, & animals not present
- comments: observed the wall exhaust fan in the rear southwest area of the premises not protected. Instructed to pest proof the rear exhaust fan to prevent pest entry and maintain.

49. Non-food/food contact surfaces clean
- comments: 4-602. 13--instructed to detail clean and maintain the following: interior and exterior surfaces of all coolers and freezers with excessive grease and dried food debris, interior and exterior surfaces of all display cooler, prep and storage racks and shelves, sides of all cooking equipment (oven, stove, mixer etc), interior and exterior surfaces of all handsink.

- comments: 6-501. 19--observed washroom door in rear prep area not self-closing. Instructed to provide a self-closing door device and maintain.

- comments: 6-201. 11--instructed to regrout floor tiles in the rear prep area underneath prep tables that have standing water in between floor tiles and detail clean floors along walls and in all corners, and behind, under, and around all heavy equipment.

- comments: 6-501. 114--instructed to detail clean and reorganize rear storage shelving and inside rear walk-in cooler currently used for non-food storage. Remove all unnecessary items and equipment stored on premises due to potential pest harborage.

59. Previous priority foundation violation corrected
- comments: previous priority foundation violation #38 not corrected from inspection report #2607153 dated:11/08/2024. We continue to observe evidence of pest activity on the premises;we continue to observe evidence of cockroaches: 3 live cockroaches underneath cracks and crevices of the front pastry display case,4 live cockroaches on the floor and underneath the 2 center tables in the rear prep area,1 live cockroach underneath the window ledge inside the washroom located directly next to the rear prep area and 1 live cockroach crawling on side of the stand-up cooler in rear prep area and 1 live cockroach crawling inside the door of the oven in same prep area and 2 live cockroaches on the walls around the storage shelf near the rear mop sink area also,approximately 45 rodent (mice) droppings on ledge behind the north grocery shelving units and on top of the east storage shelf in the same grocery customer area. Instructed to remove all evidence of pest activity on the premises and detail clean and sanitize all affected areas in detail. Additional pest control service is needed to eliminate the pest activity. Priority violation 7-42-090,citation issued.
