- comments: observed food items stored on floor in walk-in cooler and rear dry storage. Instructed manager to elevate all food six inches off floor.

- comments: observed cutting board on main butcher line covered with deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable surfaces.

- comments: observed evidence of smoking on site. Observed several cigarette butts on premises especially in the rear storage areas and by the ice machine. Instructed to remove cigarette butts and maintain. Also informed smoking is prohibited in food establishments due violation not adhering to the clean indoor air ordinance. Citation issued 7-32-015 (c)
