1. Person in charge present, demonstrates knowledge, and performs duties
- comments: no person in charge was present at the time of inspection. Priority foundation violation. No citation issued

2. City of chicago food service sanitation certificate
- comments: noted no certified food manager on duty with the city of chicago sanitation certificated or proof of while tcs foods (chicken, hamburgers etc. ) were being prepared and served. Instructed management that a certified food manager must be present at all times tcs foods are prepared and served. Also, the person in charge must have a city of chicago certificate by the next inspection (upon being inspected) priority foundation violation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: noted no soap at handwash sink of the kitchen prep area. Instructed to have soap and paper towels at all handwash sinks. Priority foundation #7-38-030(c). Citation issued.

- comments: noted no sanitizer test strip kit on-site at the time of inspection. Instructed to have a test strip kit on-site at all times to monitor the strength of the sanitizer properly. Priority foundation #7-38-005. Citation issued.

55. Physical facilities installed, maintained & clean
- comments: instructed to clean and maintain the floor drain underneath the 3 compartment sink, floors underneath and around cooking equipment at the kitchen prep area, and floors along the wall base of the bar.

56. Adequate ventilation & lighting; designated areas used
- comments: noted heavy dirt and grease accumulation inside, on pipes/conduits, and edges of the exhaust hood and filters above cooking equipment. Instructed to detail clean and maintain hood, filters, and pipes/conduits.
