Among the observations from inspectors:

- Multiple live and dead cockroaches of different sizes on the wall, floor and under the stove counter in the kitchen. Two dead cockroaches on the dining room floor. The person in charge told inspectors the restaurant doesn’t have a pest control service contract.
- In-use rice scoops were “stored in a container with cloudy water at room temperature.”
-Multiple foods didn’t meet proper cold holding temperatures.
- A handwashing sink in the service area didn’t have hot and cold running water and handwashing sinks in the restrooms didn’t have hot running water.
- Three employees didn’t have valid health certificates and the person in charge didn’t demonstrate knowledge of the Guam Food Code.
