- comments: 6-501. 11 observed several ceiling tiles missing throughout the facility, including in the basement dish area and the upstairs prep area. Instructed manager to replace all missing ceiling tiles.

- comments: observed previous priority foundation violation not corrected. Report #2597410 dated july 12 2024 instructed facility to service all areas affected by pests. Observed more than 25 small flies throughout facility; observed 1 live roach on site. Instructed manager to corrected previous priority foundation violation. Priority violation 7-42-090 citation issued.
