1. Person in charge present, demonstrates knowledge, and performs duties
- comments: the person in charge does not have a city of chicago food manager certificate. Instructed management that the person in charge must have a valid city of chicago food manager certificate. Priority foundation violation.

2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on the premises while tcs foods (chicken, potatoes, burgers) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation violation, 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handwashing sink in kitchen prep area blocked by mop bucket with containers placed inside, and handwashing sink at the cookline blocked by trashcan during the inspection. Instructed to always make handwashing sinks accessible at all times to allow for proper handwashing. Chef removed items from the handwashing sinks during the inspection. Priority foundation violation, 7-38-030(c). Citation issued.

- comments: observed no paper towels at the front prep area, handwashing sink, and no paper towels at the front host station. Instructed management that all handwashing sinks must be equipped with a hand drying provision at all times; instructed to provide. Priority foundation violation. Priority foundation violation, 7-38-030(c). Combined with previous violation. Paper towels provided at the time of inspection.

- comments: observed no handwashing cleanser at the handwashing sink in the front host station. All handwashing sinks must be equipped with handwashing cleanser at all times. Instructed to provide. Priority foundation violation 7-38-030(c). Combined with previous violation. Cleanser was provided at the time of inspection.

16. Food-contact surfaces: cleaned & sanitized
- comments: found low-temperature dish machine in the bar not sanitizing properly. Chlorine reading 0 ppm. Dish machine tagged; instructed management not to use until repaired. Must contact cdph for reinspection when the dish machine is repaired and properly maintains a sanitizer reading between 50-200 ppm. Priority violation, 7-38-025. Citation issued.

- comments: observed food storage containers in the dry storage not labeled. Instructed to label all food storage containers with the common name of food item.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit on site for the chlorine sanitizer (bleach) used for dish machine sanitizing. Informed person in charge of a test kit that accurately measures the concentration of chlorine sanitizer solution must be provided. Priority foundation violation, 7-38-005. Citation issued.

- comments: observed dust accumulation on the fan guards in the walk-in cooler. Instructed to clean and maintain.

- comments: observed a slow drainage at the handwashing sink in the rear prep area next to prep sink. Instructed to repair and maintain.

- comments: observed wet mop stored in the bucket in the rear prep area. Instructed to hang mops when not in use to allow to dry.
