Report date: Oct 16, 20252. City of chicago food service sanitation certificate
- comments: observed facility without a valid city of chicago food service sanitation certificate posted onsite for food employees. Informed person in charge city of chicago original food service certificate must be posted along with the certified manager on site when time and temperature control for safety(tcs) foods are stored. Priority foundation violation #7-38-012. Citation issued.

- comments: observed the ice bin interior in need of cleaning. Instructed to remove ice, clean and maintain at all times.
25. Consumer advisory provided for raw/undercooked food
- comments: observed menu with incorrect footnote reminder statement on the menu and missing disclosure by astericks for related menu items. Instructed to provide both disclosure statement and reminder on the menu for raw or uncooked animal-based food items offered (ex. , burgers, eggs, steak, cevich, etc. ). Priority foundation violation #7-38-005. Citation issued.

- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times.

- comments: observed milk crates being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed aluminumfoil used as liner on food prep equipment and tables. Instructed to remove.

- comments: observed the interior of the reach in coolers in need of cleaning. Instructed to detail clean.

- comments: observed facility without service sink, facility indicated they throw mop water(wastewater) outside, or in toilet. Instructed to stop this practice and a service sink must be installed.

- comments: observed in dish 3 compartment sink middle basin leaking water, instructed to repair or replace.

Instructed to detail clean under all shelving units, interior of prep coolers, hot-holding units, prep tables and deep fryers to remove spills, food debris, dirt buildup and/or grease. Must maintain at all times.

- comments: observed door leading to rear prep framing is falling off, instructed to repair and maintain
