- comments: observed 2-door cooler inside dining area with missing thermometer. Instructed to provide and maintain.

- comments: observed evidence of pest on site in the lunchroom/activity room for under the stairwell along the wall in the corner scattered on the floor where lunchroom tables are located as well as on electrical lining in said space. Instructed to clean and sanitize. Must eliminate pest activity. Priority foundation violation #7-38-020 (a). No citation issued.
