- comments: observed over 100 mice droppings scattered on floor along walls and corners in teacher lounge, chemical storage, custodial storage in rear lunchroom area, along walls in lunchroom, along walls under drainbord in kitchen and shelving in dry storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed 1/4" gap at bottom left corner of right door at exit #2 and bottom left corner of exit door #5. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observed single service utensils not inverted in serving area. Instructed manager to invert all utensils to protect from contamination.
