1. Person in charge present, demonstrates knowledge, and performs duties
- comments: the person in charge during the inspection does not have a city of chicago food service sanitation certificate. Priority foundation violation.

2. City of chicago food service sanitation certificate
- comments: no valid city of chicago food service sanitation certificate posted on the premises or such a person on site. Priority foundation violation 7-38-012 citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: no cold water available at the bar handwashing sink and the hot water noted at 125. 9f. Must provide hot and cold running water. Hot water only must not exceed a maximum temperature of 120f. Priority foundation violation 7-38-030(c) citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: interior upper water reservoir of the ice machine with black slime substance dripping down into the ice for consumption. Instructed to clean and sanitize all areas of the machine. Priority foundation violation 7-38-005 citation issued.

- comments: no metal stem thermometer on the premises to monitor hot and cold food temperatures. Must provide. Priority foundation violation 7-38-005 citation issued.

- comments: all bulk food containers not in the original container must be labeled with the product common name.

- comments: must install a splash guard between the pizza prep hand washing sink and the pizza prep table.

- comments: all stored items throughout the bar must be elevated from the floors.

- comments: noted cracked, damaged and missing tiles on the pizza dough prep station. Must repair to be smooth and cleanable.

- comments: bar hand washing sink and bar three compartment sink found extremely dirty with excessive dirt, rust and black slime. Must clean and maintain all. ----must not use cardboard as liner for shelving in the kitchen. ----noted dirty shelving throughout the kitchen. Must clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: no cold water available at the bar three compartment sink. Must provide both hot and cold running water.

55. Physical facilities installed, maintained & clean
- comments: floors in the following areas with dirt and debris: throughout the bar, behind and along the sides of kitchen coolers, under the kitchen three compartment sink and under the pizza oven. Must clean and maintain all. ----noted expanding spray foam being used as sealant on walls and surrounding piping under the pizza oven is uneven, bumpy and unable to clean. Must remove or make smooth and cleanable.

56. Adequate ventilation & lighting; designated areas used
- comments: inoperable ventilation in the west washroom. Must replace/repair.

57. All food employees have food handler training
- comments: no proof of training or food handler training for all required employees. Must provide.
