- comments: 2-101. 11 observed the original city of chicago food service sanitation manager's certificate not on site. At the time of the inspection a photocopy of the city of chicago food service sanitation manager's certificate was presented. Instructed to always post the original/valid city of chicago food service sanitation manager's certificate in plain view. Priority foundation 7-38-012.
36. Thermometers provided & accurate
- comments: 4-302. 12 observed no metal stem thermometer on site to take temperatures of cold and hot foods. Instructed to always have a metal stem thermometer on site that takes temperatures of cold and hot foods. Priority foundation 7-38-005.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 5 rodent droppings in the basement under dry storage shelves. Observed about 10 rodent droppings in the utility closet on the 1st floor. Instructed to clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation 7-38-020(a).

- comments: 4-601. 11(c) observed waterstained ceiling tiles in the rear food prep area. Instructed to replace ceiling tiles.
52. Sewage & waste water properly disposed
- comments: 5-402. 13 observed wastewater leaking from the wall and from the wastewater pipe in the basement and wastewater on the floor in between those two leaks. Instructed to repair so that wastewater is convey properly and does not leak out of the wall and wastewater pipe.

- comments: 5-501. 15 observed two overflowing outside dumpsters outside of the gated outside dumpster area and excessive garbage on the ground inside of the gated outside dumpster area. Instructed to never allow outside dumpster garbage to overflow and never allow garbage to accumulate on the ground.

- comments: 6-501. 114 observed old equipment (2 door refrigerator) not turned on. Instructed to remove old equipment or clean.

- comments: 6-304. 11 observed no mechanical ventilation above a stove/oven unit that is located in the rear food prep area. Instructed to provide mechanical ventilation or remove stove/oven unit.
