- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no adequate supplies (disposable mop, disposable latex gloves, disposable mask, and adequate sanitizer *must eliminate norovirus*) on site to properly clean up any vomit or diarrhea events that can potentially occur inside of the food establishment. Instructed to provide adequate supplies for vomit and diarrhea clean up. Priority foundation violation #7-38-005.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing sign in washroom. Instructed person in charge to provide and maintain handwashing signs in all washrooms.
38. Insects, rodents, & animals not present
- comments: observed approximately 100 rat and mouse droppings scattered throughout the facility. Instructed person in charge to remove all rat and mouse droppings and sanitize all areas as follows; walk in cooler under shelving by door, pallets located in rear of cooler, shelving in hallway near office area. #7-38-020(a). Citation issued.

- comments: observed build up on shelving in storage areas. Instructed person in charge to clean and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in women's restroom. Instructed person in charge to provide a covered receptacle and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside trash to be overflowing, incapable of fitting into dumsters on site. Instructed person in charge to remove all trash, acquire larger dumpster and maintain area. #7-38-020(b). Citation issued.
