- comments: observed ice machine used for consumption unclean with black and pink mildew-like substance inside areas of the ice machine where ice is made and stored. Must clean and sanitize the interior of the ice machine and maintain it. Priority foundation violation 7-38-025

39. Contamination prevented during food preparation, storage & display
- comments: observed ice machine containing ice used for consumption and soda syrups in mechanical room of basement with unfinished ceiling, walls, and floors. Observed exposed pipes and a sewer line not protected or shielded from possible drips above the ice machine and soda syrups. Move ice machine, soda syrups, food, and beverages out of the mechanical room and relocate into an area with smooth, easily cleanable walls, ceilings, and floors. If ice machines cannot be relocated, install smooth, easily cleanable floors, walls, and ceiling under and around ice machines and soda syrups. Maintain.

- comments: observed ice scoop stored in room temperature bucket of water next to ice machine. Do not store ice scoop in bucket of water. Store ice scoop in a clean, dry, protected container and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed basement on premises not being maintained. Observed old bowling balls, old bowling shoes, old construction debris, and old equipment stored with food and food-related products. Observed an accumulation of construction debris in 2 unfinished washrooms in the basement. Remove all debris and all items no longer needed to avoid harboring of insects and rodents. Store all remaining items at least 6 inches off the floor and away from walls for pest control, monitoring, and regular cleaning. All food and food-related items must be stored separately in the finished side of the basement away from all non-food-related items. Maintain basement.

- comments: observed basement floors unclean with dirt, dust, and debris containing liquor, beer, dry storage, and chest freezers. All basement floors need to be cleaned in detail and maintained at a frequency to prevent the accumulation of dirt, dust, and debris. Maintain.

60. Previous core violation corrected
- comments: observed previous core violations from report #2595501 dated 5/31/24 not corrected. 6-501. 12 observed excessive debris and glass shards as well as spillage from leaking soda syrup containers on the floor in the basement alcohol storage room next to the ice machine. Instructed manager to clean and maintain areas free of spillage, excessive debris, and broken glass shards. Priority foundation violation 7-42-090
