- comments: person in charge unable to provide procedure for employee to follow when responding to vomit and diarrheal events. Must provide procedure and maintain. Priority foundation violation 7-38-005 no citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signs or posters near hand sinks in women staff washrooms. Must provide and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste reeptacles in women staff and girls washrooms in main building and annex building. Must provide and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: outside garbage area not maintained. Observed outside garbage dumpsters overflowing preventing lid from closing properly. Also observed excessive amount of garbage and trash on ground surrounding containers. This is harborage and a food source for rodents and other pests. Priority foundation violation 7-38-020(b).

- comments: observed dust build up inside light shields covers in boys and girls washrooms. Must clean and maintain.
