- comments: noted interior of ice bin in need of cleaning. Instructed to remove ice, clean, sanitize and maintain at all times.

- comments: noted stained and deeply cut cutting board at boba station. Instructed to clean, sanitize and restore to a smooth cleanable surface or replace.

- comments: noted cardboard and bubble wrap used as shelf liner on storage rack between reach in cooler and prep station. Instructed to use nonporous surface that is smooth and easy to clean.

- comments: noted unused equipment in 1st floor storage, clutter in kitchen behind door next to mop sink and boxes on the floor in basement. Instructed to remove all unnecessary items, elevate brooms and store mops inverted, and provide shelves a minimum of six inches from the ground for kept items to prevent pest harboage.

- comments: noted vents above food prep area with heavy dust and walls and ceiling behind and above chest freezer in basement in need of repair. Instructed clean and maintain free of dust and to restore walls and ceiling to a smooth cleanable surface.

- comments: noted no certified food manager with allergen training. Instructed any persons with the city of chicago food sanitation certificate to comply with completion of training. Must provide documentation and maintain on premise at all times.

60. Previous core violation corrected
- comments: noted continued non-compliance from report #2561771 conducted on 8/5/22. Previous core violations not corrected: (37) food must be labeled with a common name in the coolers and storage areas. Specially food removed from the original container (38) out opening is not protected. Bottom front door with an 1/2' to 1' gap. Repair issue. To minimize pest and insect entry (41) must store wiping cloths in prep areas in a sanitizer bucket between uses to prevent cross-contamination. Instructed to maintain. (49) debris at interior and exterior of equipments,must clean and maintain (55) debris on floors throughout the premises,specially under three compartment sink,along baseboard wall,behind cooking equipments ,etc. Must keep floor dry throughout. (55) broken plastic baseboard and walls at rear area/under handwashing sink. Replace/repair. Surface must be smooth cleanable. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
