5. Procedures for responding to vomiting and diarrheal events
- comments: no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that is effective against norovirus). Priority foundation violation 7-38-005.

38. Insects, rodents, & animals not present
- comments: observed evidence of pest activity on the premises. Approximately 45 rodent (mice droppings) in the following areas on the floors underneath the storage racks inside the dry storage room and inside cluttered rear utility mop sink room and on the floor behind the kitchen 2-door stand-up cooler also,inside the managers office on floor behind the printer cabinet. Instructed to clean in detail and remove all rodent droppings from all affected areas. Engineer has been made aware at this time. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).

55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean ceiling tiles over the food serving lines to remove accumulation of dusts and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed burnt-out light bulbs in the following kitchen areas at kitchen exhaust hood and near the rear dry storage room. Instructed to provide and maintain adequate lighting at all times.
