- comments: obbserved no city of chicago food sanitation certificate posted on the premises. Management instructed that a certified food manager must be present at all times while tcs foods are being prepared and served. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no written employee health policy on the premises. Management instructed to have a signed employee health policy for all food handling employees. Priority foundation violation 7-38-010. Citation issued.

- comments: observed no hand soap provided at the exposed hand washing sink at the front desk coffee prep station. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed live small flying insects in the food prep area. Approx. 30 live small flying insects were observed on the ceiling, walls, and equipment in the food prep and dishwashing areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed no irreversible registering temperature indicator to test the surface temperature of utensils placed inside the high-temperature dish machine. Management instructed to provide. Priority foundation violation 7-38-005. Citation issued.

- comments: clean the floor beneath the bar equipment. Clean the floor drain behind the bar. Clean the food debris splash on the wall above the prep cooler in the food prep area.
