39. Contamination prevented during food preparation, storage & display
- comments: observed interior of ice machine with build up at ice chute. Instructed to clean and maintain clean at all times.

40. Personal cleanliness
- comments: observed a male employee food handler not wearing a hair restraint. Instructed manager to have employees in the food prep area to wear a hair restraint. Maintain same.

41. Wiping cloths: properly used & stored
- comments: observed wiping cloths not properly stored when not in use. Instructed manager to store wiping cloths in sanitizing solution (must be at the required ppm) when not in use. Maintain same.

- comments: observed worn sponges at 3 compartment sink area. Sponges must be changed more often. Instructed to correct and maintain.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed large can opener in need of cleaning to remove build up. Instructed to clean and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed the trash receptacle in the staff's toilet room with no lid. Instructed manager to provide a trash receptacle with a lid to the unisex staff toilet room. Maintain same.

55. Physical facilities installed, maintained & clean
- comments: observed the floors in the corners and under/around the heavy equipment in the storage, dish washing and prep area with food/dirt and debris. Instructed manager to clean and maintain floors. Also, must clean the vents in the dish washing area. Remove thick dirt and dust build up.
