- comments: observed no written employee health policy on the premises. Instructed to provide and maintain signed employee health policy for each employee working on site. Priority foundation 7-38-010, citation issued.

- comments: observed gap opening approx:1/4" inch at bottom rear exit door. Instructed to make door tight fitting all across the bottom to prevent pest entry and maintain.

- comments: observed evidence of pest activity on the premises. Observed approximately 20 rodent (mice) droppings inside cluttered rear furnace/hot water heater room. Also, observed approx; 10 rodent (mice) droppings on the floor underneath storage racks in rear dry storage room. Instructed to detail clean and remove all rodent droppings. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a), citation issued.

- comments: observed a torn door gasket inside of the rear 3 door stand-up cooler. Instructed to replace the torn door gasket and maintain.

- comments: observed accumulated grease and food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers. Instructed to detail clean and maintain.

- comments: observed dirty floor with debris underneath the dishwashing 3-compartment sink area and under and around coolers and cooking equipments. Instructed to detail clean and maintain at all times. Instructed to detail clean ceiling/walls in the front prep area and attached ceiling ventilation covers to remove accumulation of dusts/debris and maintain.

- comments: instructed to detail clean the kitchen exhaust hood/filters to remove accumulation of grease build-up and maintain.

- comments: previous core violations not corrected from inspection report#2610955 dated: 01/23/2025 #55---6-501. 114: **c** maintaining premises, unnecessary items and litter observed excessive clutter in the boiler room/storage area. Instructed to clean and discard unnecessary equipment. #58---2-102. 13: **c** allergen training - all certified food service sanitation managers employed by a restaurant must receive or obtain training in basic allergen awareness principles within 30 days after employment and every 3 years thereafter. Training programs must be accredited by the american national standards institute or another reputable accreditation agency under the astm international e2659-09 (standard practice for certificate programs). Observed no allergen training certificate for certified food manager(s). Instructed to provide. Instructed to comply with noted violatons and maintain by 6/3/2026. Priority foundation violation 7-42-090, citation issued.
