38. Insects, rodents, & animals not present
- comments: observed live pest activity on the premises. Noted large roach and a cluster of approximately 40 small roaches on side of prep cooler. 2 large roaches in circuit breaker, approximately 7 large roaches in rear hood/filter system. Management instructed to clean affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted microwave at cooksline and rear prep area not clean. Instructed to deep clean and maintain.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit for chlorine sanitizer used in sanitizing buckets and at 3 compartment sink. Informed person in charge a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation combined with above.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no irreversible test strips/testing device on site to adequately measure the utensil surface temperature (160f) after the final rinse for high temp dish machine. Instructed to provide. Priority foundation violation #7-38-005. Citation issued.

49. Non-food/food contact surfaces clean
- comments: observed grease buildup on cooking equipment, walls, surrounding floors and shelving units in the kitchen/prep areas. Instructed to deep clean by degreasing and sanitizing all surfaces

- comments: observed cold water handle at cooksline handwash sink in disrepair. Noted handle stiff. Instructed to repair to ensure proper handwashing
