Management instructed that a certified food manager must be present at all times with their original city of chicago food sanitation certificate while tcs foods are being prepared or served. Priority foundation violation 7-38-012. Citation issued.

- comments: observed discoloration and mold-like growth on the interior contact surfaces on the ice machine located in the food prep area. Management instructed to clean and sanitize all interior surfaces of the ice machine. Priority foundation violation 7-38-005. Citation issued.

- comments: observed edamame beans and raw beef thawed at room temperature in the front prep area on top of the prep table and prep cooler. Management instructed to thaw all tcs foods properly.
