- comments: observed blank copies of employee health policy on site. Instructed facility to maintain policy with verifiable signed copies on site for all food employees in acknowledgment of reporting agreement. Priority foundation violation #7-38-010.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed a white panel inside the ice machine that touched ice filled with a black mold-like substance. Instructed manager to wash, clean, sanitize and maintain. Ice is used for human consumption. Priority foundation violation. 7-38-005. Citation issued.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed the improper chemical test kit available during inspection. Instructed manager to provide the correct (quat) test kit for proper concentration in sanitizing solutions. Priority foundation 7-38-005 citation issued.
49. Non-food/food contact surfaces clean
- comments: observed an excessive amount of ice buildup on the floor/walls/ceiling of the walk-in freezer. Must remove excess ice, detail clean, and maintain. Also, observed food debris on the floor of the walk-in cooler. Instructed to clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed missing ceilings tile panels in the rear. Instructed to replace missing panels ceiling tiles.
56. Adequate ventilation & lighting; designated areas used
- comments: observed a burned out light-bulb in the hallway where the ice machine is stored. Instructed to replace bulb and maintain lighting in the unit.
57. All food employees have food handler training
- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.
58. Allergen training as required
- comments: observed the city of chicago food certified manager with no proof of allergen training certificate. Instructed all city of chicago certified food managers obtain this certificate.
