- comments: dead cockroaches on floor in rear dish-washing area. Must clean floors, remove all dead roaches.

- comments: exterior of dish-machine not clean. Must clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed leak from ceiling and and standing water on floor in basement sump pump room. Must repair leak and remove standing water.

59. Previous priority foundation violation corrected
- comments: 8-404. 13(b)(3) previous priority foundation violation from report #2510762 on 6/3/21 not corrected: #38 - still live cockroaches. Observed 6 live cockroaches on the floor in dish washing area and 1 on side of garbage container. Additional pest control service is needed to eliminate the pest activity. Priority violation 7-42-090.
