Report date: Dec 13, 202210. Adequate handwashing sinks properly supplied and accessible
- comments: at time of inspection hot water sink fixtures in the following areas do not have hot water and have inadequate hot water pressure. Hand sinks in classrooms 2-6yrs; restrooms outside classroom areas 2-6yrs/infant/toddler; restroom located near office. Temperature measured at approximately 70�f. During exit interview, hot water was provided at handsink outside of office area. Provide water of at least 100f through a mixing valve or combination faucet. Priority violation 7-38-030 (c) citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: a sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks in facility and sink units that are used by food employees. Provide a clearly visible handwashing sign at each hand sink.
36. Thermometers provided & accurate
- comments: thermometers are not provided in the small cooler located near the drinking fountain. Cold/hot holding equipment used for time/temperature control for safety shall have at least one thermometer in an easily viewed location.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: test kits/ test strips or other devices are not available to the employees to measure sanitizing solution concentration. The manager provided test strips for drinking water and not for chlorine that is used to sanitize dishware. The facility must provide a test kit or other measuring devices for the employees can measure the sanitizer concentration and avoid a concentration that is too high, which can be toxic. Priority foundation violation 7-38-005 citation issued. Test strips were provided during the exit interview. The citation was voided.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: warewashing 3-compartment sink does not have a drain board or section to air dry dishware/utensils after they are sanitized. Facility must provide a station to air dry sanitized dishware/utensils at the 3compartment sink.
51. Plumbing installed; proper backflow devices
- comments: observed a backflow prevention cap that is completely broken and water is dispensing out of backflow device, in the kitchen area. Discussed with manager that backflow prevention device must be installed properly and facility must maintain maintenance.

- comments: observed leak of water under the 3 compartment sink. The facility is also using black tape used around the pipe under the 3-compartment sink. The facility must repair and maintain.

- comments: all restrooms in the facility does not have a tight-fitting and self-closing door. All toilet rooms located within the establishment must be enclosed with a tight-fitting and self-closing door.

- comments: restrooms lacks a covered receptacle. Facility must provide at least one covered trash receptacle in each restroom.
54. Garbage & refuse properly disposed; facilities maintained
- comments: the area around the outdoor waste receptacle is heavily soiled, with garbage overflowing in dumpsters on and around the ground, creating a nuisance and pest attraction. Outside receptacles and receptacle areas must be cleaned and maintained. The owner/staff have cleared garbage on top of the dumpster bin. Priority foundation violation 7-38-020 (b) citation issued.

- comments: waste receptacle not found near the handwashing sink located in kitchen and classrooms (2-6yrs). When disposable towels are used at handwashing sinks, a waste receptacle shall be located at each handsink in the facility.

- comments: observed missing caulking located at all sink fixtures in the facility. Areas include but is not limited to; exposed handsinks, restroom handsinks, fountain drink station, and mop sinks. Facility must ensure to caulk sink untis adjacent to the walls at all sink fixtures throughout the facility and must ensure all surfaces are smooth, easily cleanable, and non-absorbent.

- comments: grout/base coving is missing along floor adjacent to wall in restroom located across water tank heater/mop sink. Facility must ensure all surfaces are smooth, easily cleanable, and non-absorbent.

- comments: unnecessary items / nonfunctional equipment found in storage closet near water tank heater and observed a 3compartment sink no longer in use located near 2-6yr old classrooms. The premises shall be free of items unnecessary to the operation or maintenance of the establishment. This includes equipment that is nonfunctional or no longer used.
