- comments: no employee health policy on premises as required; instructed to provide and maintain. Instructed the person in charge to provide documentation of employees responsibility to report any illness transmissible through food. Priority foundation violation 7-38-010 citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed the employee washing dishes in the three comp sink without a chemical sanitizer as required. The three comp sink was not set up properly to wash rinse and sanitize. Instructed to correct and maintain at all times. Priority violation 7-38-025 citaiion issued
25. Consumer advisory provided for raw/undercooked food
- comments: no consumer advisory, disclosure or reminder found on printed menus as required; instructed to provide and maintain. Priority foundation violation 7-38-005 issued.
38. Insects, rodents, & animals not present
- comments: found no pest control log book from company being used by current owner; instructed to provide and maintain. Priority foundation violation 7-38-020(c) citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed loose rubber gaskets at cooler doors; instructed to replace where needed. Observed the interior panel of the middle door at the three door prep cooler missing; instructed to replace panel.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: found no chemical test strips on premises as required; must provide and maintain. Instructed the person in charge to provide chemical test strips for the three comp sink and dishmachine on site to test the strength of the sanitizer when wash rinse and sanitizing. Priority foundation violation 7-38-005 citation issued.
49. Non-food/food contact surfaces clean
- comments: instructed to clean the bottom of the fryers and exterior surfaces of the fryers and grill, clean the blade of the can opener in the rear prep area, clean area under grill between grill and table. Clean interior surfaces of refrigeration units.
50. Hot & cold water available; adequate pressure
- comments: found no hot running water at the following locations: exposed handsinks in prep area, three comp sink in dishwashing area and washrooms. Instructed to provide and maintain at all times. Priority violation 7-38-030(c) citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed some ceiling tiles with water stains in the dining area; replace tile. Observed some missing ceiling tile in rear area; intructed to replace where needed. Observed peeling paint on the walls of the basement storage area; instructed to remove and make walls smooth and easily cleanable. Instructed to clean the floors around under and behind equipment where needed and in basement area.
57. All food employees have food handler training
- comments: instructed that all food handlers must obtain the required food handler training.
58. Allergen training as required
- comments: instructed that all city of chicago food service managers must obtain the required food allergen training.
