- comments: observed no written employee health policy provided on the premises. Management instructed to provide a signed employee health policy for each food handling employee. Priority foundation violation 7-38-010.
38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Observed approx. 175 rodent droppings located in the following areas: 5 droppings under the steam tables in the food service line, 5 droppings on the floor and storage shelf in the dry storage room, 5 droppings on the floor in the student dining area, 10 droppings on the floor behind the grease trap and 3-compartment sink in the food prep area, 10 droppings on the floor under the 2-compartment prep sink in the food prep area, 10 droppings inside of the unused hot holding cabinet in the food prep area, 10 droppings on the floor behind the stove in the food prep area, 50 droppings on the floor and storage shelves in the mop closet located in the student dining area, 50 droppings on the floor of storage room 010a, and 20 droppings on the floor in the pipe chase located next to the dining area. Management instructed to clean and remove all droppings throughout the premises. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).

- comments: previous core violation from report 2613045 on 3/4/25 not corrected: 53 - observed no covered waste receptacle at the unisex washrooms located in the lunchroom and kitchen staff washroom. Must provide covered waste receptacles in all female and all gender washrooms and maintain. Priority foundation violation 7-42-090.
