- comments: observed no thermometer conspicuously placed in the two-door cooler located in the rear outdoor storage area. Instructed the person in charge to provide thermometers inside all cooling units.

- comments: observed no splash guard between the exposed hand sink and the three-compartment sink located in the rear preparation area. Instructed the person in charge to install a washable splash guard to prevent contamination.

- comments: observed cutting board with deep scratches and stains on the prep table in the rear preparation area. Instructed the person in charge to resurface/replace the cutting board and maintain it.

- comments: observed some grease build up and food debris on surfaces of all cooking equipment located in the rear preparation area. Instructed the person in charge to detail clean and maintenance.

- comments: observed multiple holes in the ceiling in the basement above the dry storage shelving unit. Instructed the person in charge smoothly to seal holes and maintain them in good repair.

Instructed the person in charge to detail clean, sanitize and increase the cleaning frequency of floors.

- comments: observed missing and damaged floor tiles in front of the low-temperature dish machine in the rear preparation area. Instructed the person in charge to repair damaged floor tiles to create a smooth and easily cleanable surface. Must maintain at all times.

- comments: observed two certified food managers with expired allergen awareness training certificates. Instructed the person in charge to renew expired allergen awareness training certificates and maintain them.
