Violations:
- High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 2 live flies flying in the ware washing area and landing on package foods.

- High Priority - Roach activity present as evidenced by live roaches found. Observed 3 live roaches under the 3 compartment sink.

- High Priority - Roach excrement and/or droppings present. Observed approximately 15+ roach droppings under the 3 compartment sink, 5 roach droppings on top of the prep table by the 3 compartment sink, approximately 10+ roach droppings behind the reach in cooler in the kitchen.

- High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 15+ rodent droppings by the storage rack in the ware washing area, approximately 3 rodent droppings in the cabinet at the front counter.

- High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed pork (48F - Cold Holding); duck (55F-63F - Cold Holding); fresh garlic in oil (53F - Cold Holding); whole chicken (53F - Cold Holding) stored in the reach in cooler, as per operator unit has been opening and closing all day. Observed duck (73F - Cold Holding/2nd temp 60F) stored on top of the 3 compartment sink to dry, as per operator placed 2 hours before inspection, operator placed items inside the reach in cooler.

- High Priority - Time/temperature control for safety food controlled by time and required to be marked with the use by time is not marked and the required time of disposal cannot be determined. See stop sale. Observed pork ribs (98F - TPHC); roasted duck (102F - TPHC); chicken (104F - Reheating) stored at the front counter hot unit. As per operator placed items at 12pm never wrote time on sheet.

- High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. See stop sale. Observed pork ribs (98F - TPHC); roasted duck (102F - TPHC); chicken (104F - Reheating) stored at the front counter hot unit. As per operator placed items at 12pm never wrote time on sheet.
