- comments: observed bare-hand contact with ready-to-eat foods, such as slicing of limes, by a male bartender. Instructed to use gloves or utensils to handle ready-to-eat foods. Appx. 1lbs. Sliced limes. (priority 7-38-010)(cos)(citation issued)

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing sign at the hand sink in the first toilet. Room must be provided and maintained.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed a low temperature dish machine in use sanitizing at 0. 0 ppm's. A held for inspection tag was placed on the unit. Must repair and maintain. Instructed to use 3- compartment sink or additional dish machine on 1st fl. (priority 7-38-025)(cos)(citation issued)

- comments: observed improper temperature of 3lbs. Cooked grilled onions ranging from 70. 5f-90. 0f on topside of grill and on overhead shelf at grill. Product was discarded by manager must have hot holding foods at 135. 0f or above. Appx. 3lbs. $8. (priority 7-38-005)(cos)(citation issued)

- comments: observed vacuum packaged foods on site without a cdph-approved haccp plan. Instructed that a haccp plan must be approved before food can be vacuum packaged on premises. Observed 1. 5lbs. Parsnip. The product was discarded by management. A held for inspection tag has been placed on the vacuum package machine. Appx. 1. 5lbs. $5. (priority foundation 7-38-005)(cos)(citation issued)

- comments: must label food storage containers when food is not in original package (small containers,squeeze bottles,etc)

- comments: observed evidence of appx. 75 or more mice and rat droppings inside of fryer cabinet, on floor under counter at bar, under staircase in basement, in liquor room on shelving unit, and on floor next to mop sink, audio room. Must remove droppings, clean, and sanitize areas. Recommended to have a pest control operator to service premises on or after 12/11/25. (priority foundation 7-38-020(a)(citation issued)
