- comments: observed employee toilet rooms located outside of building on north side near park. Toilet rooms shall be conveniently located within the same building and not require employees to exit building to access toilets. Priority foundation 7-38-030(c) citation issued

- comments: observed dust buildup inside ventilation hood, ceiling and wall vents, debris on floors along walls and corners in kitchen. Instructed manager to clean and maintain.
